If your ship got destroyed, you were able to fly around as a guy in a space suit. You could return to your mothership to get a new ship, though.

guessing we needs more clues.
it is a vertical and horizontal shooter.
it's on the NES, but that's reasonably obvious.
as there's not much else to say, and putting that stuff in google doesn't really help, this one will probably give it away: it's on nintendo8.com

No, but they should release an updated version of that with high res graphics and really pretty, wide open areas in some places.

Okay, the only way to get back to your original body is to collect items from winning puzzle games.
