After a bit of research, I'm going to have a stab and say it's LSD: Dream Emulator (or just LSD, I'm not sure what the official title is). Looks craaaazy.

It is indeed crazy, you teleport to another part of the world if you bump into walls/people/creatures, textures warp into anything every time to enter a different area or sit still too long, creatures appear at random when you get further into it and the soundtrack is demented.
