I was thinking streets of rage two as well, that was the game that came with my Sega Genesis.

I can see how the stars threw you off but those are Streets of Rage 1 levels, Streets of Rage 1 end boss, Streets of Rage 2 mechanics. It also has Streets of Rage 3 characters, the ability to play levels across all three games and even some fan-made levels thrown into the mix.
This is a fully playable Streets of Rage game. It's just not an official one.
