Feed Dump episode I thought was relevant wasn't relevant. Can't find the one I was thinking of.

With the right keywords... voila!

The episode you were thinking of is probably Sausage Cozies.

"I didn't ask for this!"

And written in very tiny print near the bottom of the headstone:

No trees were killed to send this message, but a large number of electrons were terribly inconvenienced

"If you're reading this, something has gone terribly wrong. It's all in your hands now."

Maybe. I'm probably having my parts donated to medicine, then donated to science, then burned and spread in a forest somewhere, without a headstone.

I want to be cremated, and I kinda object to graveyards in general principle (unless you're eventually going to rotate that now nutrient-filled land into agriculture "If we're gonna recycle, let's get serious about it." - GC)

...but at the same time, I certainly wouldn't mind having some kind of permanent memorial that holds an epitaph from me.

I'm hoping that promission thing the Swedes are working on takes off. All the benefits of cremation, but far more environmentally friendly.

Best "permanent monument" I can think of would be a small capsule shot into space. Sure, no ones ever going to see it again, but at least it's out there.
